Connect the Dots Cohort · Signature program
Plan it, learn it, and get support while you do it.
Most groups get one piece: a planning retreat, a training, or a coach. The Connect the Dots Cohort brings all three together over several months, so your team builds the plan, gains the skills to carry it out, and has support when the work gets hard.

How it works
Three dots, one journey.
Facilitation
Your group names what’s really going on, including the things nobody has said out loud.
Planning and training
Your people build the plan together and learn the methods to lead their own meetings.
Group coaching
Support between sessions, using Lumina Spark, so the plan keeps moving after the meeting ends.
Two tracks
Built around what your group needs.
Proven with public health teams
Planning Track
Plan together, build skills, and get coached through the first months of doing the work.
Build your own facilitators
Facilitation Track
Grow people inside your organization who can lead meetings, planning and community input, with practice and coaching along the way.
Is this for you?
→ Your plan has stalled before, and you want this one to stick.
→ You lead a health department, coalition, nonprofit or team that answers to a community or board.
→ You want your own people to lead the work, not depend on an outside facilitator.
Questions people ask
How long does a cohort run?
It’s built around your goals and timeline, usually over several months. We map it out together in a Starting Point Conversation.
What does it cost?
Each cohort is scoped to your group. After a Starting Point Conversation, you get a clear proposal for the process we agreed on.
Can training sessions carry continuing education credit?
Ask us. Many of the ToP® methods taught in a cohort are part of courses that carry CHES/MCHES, AICP, PMI and ICF credit.
